What to expect from your first visit:
An initial 60-minute appointment consists of a comprehensive interview covering your personal health history, your chief concern, a physical examination, and an individualized treatment plan to address your concern.
Initial online intake forms:
After calling our office, you will be requested to set up a Patient Ally account that provides you with a secure platform to fill out intake documents.
A link will be sent to your email; you must access this link from a computer to set up a username and password. It unfortunately does not work on a tablet or phone.
Once logged on, click on the “Documents” icon or left-hand-side tab. From there, click on “Intake Documents”. Please complete all forms and wait for confirmation on the screen that forms have been successfully submitted.

Once Intake Forms are completed, please call to schedule your first appointment.
Cancellation / Reschedule Policy:
If you have questions before your appointment, please don’t hesitate to call. If for any reason you need to cancel or reschedule your appointment, we ask that you give 48 hours notice.
Insurance:
Discounted out-of-pocket rates (due at the time of service) are available for patients without coverage. We accept credit, debit, check, or cash.
We are preferred providers on the First Choice Health Network, Premera Blue Cross (Lifewise), Regence Blue Shield, Kaiser, Aetna, and United Healthcare.
